As a new eFunds associate, you will need to enroll in our benefits plan. Although this is typically done electronically, you will need to complete hard copy enrollment forms so that we may expedite this process.

Once we have your completed forms, we will then pass the necessary information to our various insurance providers in order to establish you and your dependents, if applicable, as plan participants.

Once you have reviewed this information, please print and complete the first four (4) forms listed below. The remaining forms are optional depending on your situation/elections. Did You Know?

All completed forms must be received within 31 calendar days of the closure date of the merger, unless otherwise notified.
